Request an EOP Presentation

Please Submit the Form Below to Request a Presentation

Requests must be submitted at least 3 to 4 weeks before your event. Our office will attempt to accommodate your request and respond within 5-7 business days.
All guests must receive an email confirmation from the EOP department prior to your scheduled event.
Off-campus presentation requests will be considered; we are available via virtual modality.
If your group of participants is under the age of 18 years old, the university requires that each student complete a release of liability form. Please bring signed hard copy forms on the day of your event: Liability Form for Minors.

OFFICIAL CAMPUS TOURS

Required to follow the Information for Guest in consultation with your host on campus.
Please do not attend the event if you are experiencing any COVID-19 symptoms or are under quarantine or isolation orders.
If you have a COVID-19 incident within two weeks of visiting Cal Poly Pomona, please contact healthscreener@cpp.edu immediately upon knowledge.
If your group of program participants is under the age of 18 years old, the university requires that each student complete a release of liability form: https://www.cpp.edu/rms/forms/release-of-liability-2022.pdf. This form requires a wet signature, so please have each complete and signed by the parent/guardian. We ask that the student group lead/program coordinator collect all completed forms and bring them to campus on the event date.
