If you have not taken courses with me, indicate if you are my advisee, research student, etc. Also let me know the date or approximate date when I first met you.

CV or Resume and Personal Statement if you have one.
Any relevant information that will make you a valuable participant in the program you are applying for. For example, let me know of your participation in research, work experience in science, etc. For your research/science experience, indicate who is your current/past PI mentor.
