Employment Verifications
When an employee applies for a loan, home mortgage, new job, and/or rental qualification, any requests for verification of your employment can be done two ways which are outlined below. Please note that employees may sign up for the State Controller's Office Cal Employee Connect website to view and download their paycheck earnings statements and W-2 forms.
Submit an HR Customer Service Request Form or send us a fax at 909-869-4868 with the verification request. Please make sure to include a signed authorization by the employee to be able to proceed further. Also, please allow our team a 72 business hour turnaround time to allow for the verification to be returned to the lender/employee.

Please note: This service is not available to student assistants and employees who have an hourly position in addition to their full-time/part-time position.
Public Service Loan Forgiveness Program
Use the PSLF Help Tool to confirm you are eligible, fill out, and submit your PSLF form.
The Human Resources Customer Service Center will complete Sections 3 and 4 of the PSLF document to verify that employment is with a public service organization. Employment Certification forms may be submitted:
- In-person to our office in Student Services Building 121-West-2700.
- Through our HR Customer Service Request Form. Please be sure to not include any sensitive information (Social Security number) in the form.
It is the responsibility of the PSLF applicant to submit the application, and all required paperwork, to the U.S. Department of Education in order to qualify for the PSLF Program. The U.S. Department of Education, not the CSU, reviews the applications and makes the determination of eligibility for the PSLF Program.
