Life Insurance and AD&D
The CSU provides Basic Life Insurance and Accidental Death and Dismemberment (AD&D) at no cost to you as an eligible employee. The amount of coverage varies based on employee category.

Basic Life Insurance and AD&D become effective on the first day of the following month from your date of hire or eligibility. For questions, exclusions, limitations, and reductions, beneficiary designations, and claims, please refer to The Standard.
Some features of the coverage include:
- Waiver of Premium: Premiums are paid by Cal Poly.
- Portability of Insurance (PDF): Upon separation, you may continue coverage at your cost.
- Travel Assistance (PDF): Offers aid before/during trips, including visa information, health recommendations, and connection to medical providers.
- Life Services Toolkit (PDF): Provides resources for financial planning, estate planning assistance, health and wellness, funeral arrangements.

The CSU recognizes that individuals have diverse needs and has provided eligible employees with the opportunity to apply for the right amount of protection for you and your family. This is in addition to any employer-paid basic life, accidental death and dismemberment insurance already provided.
Employees may apply for Voluntary Life and Accidental Death & Dismemberment within 60 days of their hire date with no medical certification required for up to the guaranteed issue amounts of $150,000 for employees, $50,000 for spouses/domestic partners and $20,00 for dependent children.
