Enrollment
Complete the following within 60 days of your hire date:
- Review the Health Plan Directory and find your providers/specialist by going directly to the health plan website, calling the health plan or using CalPERS Health Plan Search by Zip Code.
- Submit a Benefits Enrollment Worksheet by logging into PolyDoc using your CPP credentials.
- Submit an e-signature authorization by logging into PolyDoc using your CPP credentials.
- Create a My CalPERS account.
- Recommend waiting after your 30 days of hire to allow processing time.
- Designate your beneficiaries on My CalPERS.
- Create an account on The Standard.
- Recommend waiting after your 30 days of hire to allow processing time.
- Designate your beneficiaries on The Standard.
- Enroll in your Voluntary Plans, if applicable.
As an eligible employee, you have 60 days from the date of your eligible appointment to enroll in benefits. If you miss the initial 60-day enrollment period, you can still enroll, however, your coverage will not be effective until the first of the month following a 90-day waiting period. Your benefits will become effective the first day of the month from when you submit your benefit enrollment worksheet (if within 60 days).
To enroll in benefits, please complete a Benefits Enrollment Worksheet by logging into PolyDoc using your CPP credentials. Make sure all required documents are attached as your enrollment may be delayed if there is missing documentation. If you have trouble submitting supporting documents to PolyDoc, please try using a different browser such as Chrome, Firefox, etc. and/or clear your cache on your web browser and enable pop blockers.
Complete your e-Signature Authorization form on PolyDoc by logging in with your CPP credentials. This will eliminate the need for employees to return and sign benefit enrollment documents for all transactions.
After 60 days, an employee has the opportunity to request a Late Enrollment. Late Enrollments require a 90-day waiting period OR you may enroll during the next Open Enrollment. The earliest effective date of enrollment will be the first of the month following the 90-day waiting period or the January 1 after Open Enrollment.

A qualifying life event are significant changes in your life that allow you to make changes to your benefits outside of the Open Enrollment period. These are available for:
- Changes in Household
- Marriage or divorce
- Birth or adoption
- Death of a family member
- Changes in Residence
- Moving to a new ZIP code
- Loss of Health Coverage
- Losing employer-sponsored health insurance due to job loss, reduced hours, or the employer discontinuing coverage
- Losing eligibility for Medi Cal, Covered California, or Medicare
- Turning 26 and aging out of a parent’s plan
