Staff/Management Employees
- Appointed to at least half-time (0.50) or more, and
- An initial appointment duration of more than six months (six months and one day)
An employee who meets the following eligibility criteria is only eligible to enroll in a health plan or Flex Cash health under the Affordable Care Act (ACA):
- Appointed with at least a .75 time base and an initial appointment duration of less than six months and one day, or
- Appointed to work at least 130 hours per month
- Appointed to at least half-time (7.5 WTU's' or greater), and
- An initial appointment duration of more than six months (six months and one day).

- Appointed to at least 0.4-time base (6.0 WTU's' or greater), and
- Initial appointment duration for at least one semester.

Eligible dependents of an eligible employee include:
- Spouse (unless legally separated or divorced)
- Domestic partner: Must be registered through the Secretary of State process, (see eligibility criteria for enrolling your domestic partner (PDF)).
- Registering your Domestic Partner into Medical and Dental is considered imputed income by the IRS. Each month, an employee who enrolls their Domestic Partner into their benefit plans will have additional taxes taken out of their warrant. - If a CSU employee claims their domestic partner and/or the domestic partner’s children as a dependent, the form Domestic Partner Tax Dependent Certification Form (PDF) must be completed and signed by the employee. The original form must be retained by the employer.
- Children (younger than 26): A dependent child includes a natural born child, adopted child, stepchild, or a child living with the employee in a Parent-Child Relationship (PDF) who is economically dependent upon the employee.
- Disabled Dependent (26 or older): Follow the instructions on the Disabled Dependent Member Questionnaire and Medical Report Form (HBD-34) (PDF) and the Authorization to Disclose Protected Health Information Form (PDF).
When enrolling any dependents, you will need to provide each dependent’s:
- Social Security Number
- A copy of a birth certificate for dependent children
- A copy of a marriage certificate for a spouse
- A copy of Domestic Partnership certification for a Domestic Partner.
The Dependent Eligibility Verification (DEV), administrated by CalPERS, is the process of re-verifying the eligibility of dependents enrolled in a medical, dental, and/or vision plan once every three years based on your birth month. This is to ensure only eligible dependents of State and CSU employees are enrolled in employer-sponsored health coverage. Verification Schedule
Your family members' eligibility will be verified once every three years. The verification cycle is based on your birth month. The following chart illustrates the schedule:

How to Verify Dependents?
- Submit your Dependent Verification Affidavit via Adobe Sign.
- Submit Supporting Documents (marriage certificate, birth certificate, etc.) via PolyDoc.
What Documents are Required?
Spouse/Domestic Partner
- Marriage Certificate or Declaration of Domestic Partnership
- A copy of the first page (1040) of an income tax return from the previous tax year listing the employee and the spouse or domestic partner, or a household bill, account statement, or insurance policy listing the name and address of the employee and the spouse or domestic partner within the last 60 calendar days
Children (Natural Born, Adopted, and Domestic Partner Children)
- A copy of birth certificate naming the employee as the parent
