- Fill out a Grade Forgiveness through Course Repetition Form (located in the Forms tab).
- Take the completed form to your Academic Advisor for their signature.
- If the catalog course number has NOT changed, the Department Chair DOES NOT need to sign the form and you can take the form directly to the Registrar's Office for processing.
- If the catalog course number HAS changed (Ex. 1st attempt: BHS 204-2nd attempt: PSY 204), then the Department Chair signature is required. After, the Department Chair's signature is obtained, you can then take the form to the Registrar's Office for processing.
- NOTE: You cannot complete a Grade Forgiveness through Course Repetition Form if you received a W in the course.
- The Class Withdrawal form is now submitted via online on the Registrars website under Student Forms & Services.
- The Leave of Absence is now submitted via online on the Registrars website under Student Forms & Services.
- Fill out a Request for Award of Minor Form (located in the Forms tab).
- We recommend that you complete Part II of the form with the minor coordinator. Please meet with the coordinator of the minor that you are appying for.
