Employee Resources
Under the American with Disabilities Act and Fair Employment and Housing Act, qualified individuals with disabilities must have equal access to all aspects of employment that are available to employees without disabilities. These Acts also require employers to afford such individuals reasonable accommodations that will allow them to perform the essential functions of their jobs.
Cal Poly Pomona is fully committed to ensuring equal employment opportunity for qualified applicants and/or employees with disabilities and engages in a timely, good faith, interactive process to determine effective reasonable accommodations. Please note that all requests and supporting documentation submitted for an Employee Accommodation Request are kept in a secure location, separate from official personnel or HR files.
Employees can submit an Employee Accommodations Request one of two ways:
- Submit via Employee Request For Disability-Related Accommodation Adobe Sign OR
- Complete the Employee Request For Disability-Related Accommodation (PDF) form and either hand-deliver the completed document to the HR Customer Service Center (Student Services Building 121-Second Floor) or send via fax to 909-869-4868
Employees, who have already submitted an Employee Accommodation Request, can upload their medical provider documentation in one of two ways:
- Submit via Medical Provider Inquiry Attachment in Response to an Accommodation Request Adobe Sign OR
- Complete the appropriate PDF form and either hand-deliver the completed document to the HR Customer Service Center (Student Services Building 121-Second Floor) or send via fax to 909-869-4868
Employees with questions about disability support and accommodation should contact the ADA Coordinator who has been designated as the campus authority to verify employee disabilities and to prescribe specific accommodations for employees with documented disabilities.
- Complaints: Employees are urged to contact the Office of Equity and Compliance regarding any questions or concerns, including complaint procedures available to you. For employees represented by a collective bargaining agreement, the collective bargaining agreement outlines complaint procedures, process, and timelines. For employees not eligible to file a complaint or grievance under a collective bargaining agreement or whose collective bargaining agreement incorporates CSU system-wide omplaint procedure, the complaint procedure is outlined in CSU Executive Order 1096.
- Prospective Employees and Visitors: If you are a prospective employee or a campus visitor seeking disability support and accommodation resources, please contact the individual office applicable to the accommodation for parking issues, Talent Acquisition regarding staff interview accommodations, etc.) or contact the ADA Coordinator for guidance and further assistance.
